With our recent implementation of EvolveNXT, we are pleased to provide our broker partners with a faster, more innovative, and secure way to manage your Clever Care business.

Complete the form below to initiate the Clever Care agent/agency contracting process today. A member of the Broker Support Team will be in touch with more information. As a contracted agent, you will receive access to our Broker Portal – where you can track your Clever Care book of business, track application statuses, and view commission statements. The broker portal will also have a menu of quick links to online enrollment, marketing materials, and more.

Contracting Requirements: 

To be authorized to sell Clever Care Health Plan, you must complete the requested information.

Clever Care paperwork includes:

  • Current CA State Insurance license – You must have a current California Accident & Health license (must not expire within 45 days of contracting)
  • Signed W-9 (current year)
  • Agent Agreement
  • Errors & Omissions policy attestation – Applicants must attest to Clever Care that E&O coverage is current (must not expire within 45 days of contracting). Your minimum E&O policy must be written for $1,000,000 per occurrence and $1,000,000 aggregate limit. After onboarding, agents are required to continue to maintain active E&O coverage.
  • Assignment of Commissions – If applicable
