Announcing Clever Care’s HRA Incentive Program for AEP!
We’re thrilled to announce that a Health Risk Assessment (HRA) Incentive amount for Total+ and Breathe+ C-SNP Plans has increased to $250 through AEP 2025. This applies to new member enrollments with effective dates from November 1, 2025, December 1, 2025, and January 1, 2026.
You can earn $250 for Total+, $250 for Breathe+, and $80 for Longevity.
To be authorized to sell Clever Care Health Plan, you must complete and submit the following information to initiate the “Welcome” email.
