The Clever Care SNP Programs are designed to maximize the quality of care, access to care, and health outcomes for the SNP populations it serves especially the most vulnerable population we serve. Clever Care’s Model of Care (MOC) is vital to our ability to achieve this mission.
We invite you to review the Model of Care training presentation to find out more about the Clever Care MOC and the role you play in ensuring that Clever Care and its affiliates continue to address the unique needs of our SNP members.
