Notice of Data Security Incident

Important information regarding a cybersecurity incidentexperienced by DentaQuest, our dental benefits provider for 2025 coverage.

Introduction

Clever Care values the privacy and security of our members’ information. We are providing notice regarding a cybersecurity incident experienced by DentaQuest. DentaQuest was contracted with Clever Care Health Plan to provide your dental benefits in 2025.

This notice is intended to provide information for individuals who may have been affected by the incident and for whom direct notification may not be possible due to insufficient or outdated contact information.

What happened?

On May 20, 2026, DentaQuest identified unauthorized access to certain information stored within its computer network. DentaQuest immediately secured its systems, notified law enforcement, and engaged leading independent cybersecurity experts to investigate the incident.

The investigation determined that unauthorized access occurred between May 17, 2026, and May 20, 2026.

What information was involved?

DentaQuest’s ongoing review has determined that the information involved may vary by individual. Information potentially affected may include:

Personal Information

  • Name
  • Address
  • Social Security number

Member Information

  • Member identification number
  • Medicaid number
  • Medicare number

Health Information

  • Dental and/or vision information
  • Provider name
  • Diagnosis information
  • Treatment information
  • Billing information

What is being done?

DentaQuest has begun mailing notification letters to affected individuals and is offering 24 months of complimentary identity monitoring services through Kroll to eligible individuals. These services include:

  • Credit Monitoring
  • Fraud Consultation
  • Identity Theft Restoration

In addition, DentaQuest has implemented enhanced security measures, strengthened monitoring controls, and provided additional workforce training to help prevent similar incidents in the future.

What can you do

We encourage potentially affected individuals to:

  1. Contact DentaQuest to determine whether their information was involved.
  2. Enroll in the complimentary identity monitoring services being offered.
  3. Review account statements, explanation of benefits (EOBs), and credit reports for suspicious activity.
  4. Promptly report any suspected identity theft or fraud to the appropriate authorities.
